What is the greatest number of pitches a major league pitcher has thrown in an inning?

You often hear how, if a pitcher is having a rough inning, he is having to throw upwards of 25 pitches in order to get three outs. A pitcher is often good for about 100 pitches a game, so I'm wondering if a pitcher has ever approached that in one inning?
